2008-2009 SCHOOL FORMS
ALL students must have the following forms complete prior to the beginning of school (please do not combine sibling information on forms): Student Information Form Health Form Cover Letter (does not need to be returned) Student Health Form Administration of Prescription Medication Form Authorization for Pick-Up Form General Release Form Acceptable Use Policy Agreement Form
The following forms are only applicable for specific grade levels: After School Care Registration Form (Lower and Middle School) Alpha Acknowledgement Form (Alpha)
General School Forms include: Student Information Update Form: Please submit any changes and updates to student directory information, parent contact information or emergency contact information in writing on this form to the Registrar.
Transcript Request Form for release of student transcript and other academic information. Please submit any requests for information about students along with this form to the Registrar. Please do not give any forms to classroom teachers. When authorization has been received by the Registrar, records will be mailed from the school to the requesting school or professional. They are not given to the parents for hand delivery. This is a very strict policy which the professional educational community respects and demands.
Anticipated Absence Form: In the case of a planned absence please complete and submit this form to the Lower School office. Please contact your child’s individual teachers for homework assignments.
