Physicals and Required Forms

The Physical Examination Form is required for participation in sports at Presbyterian School.  Students are not allowed to begin practice until these forms are completed.  The Physical Examination Form is part of the 2017-18 Required School Forms distributed and collected by Magnus via the School website  Parent Portal.  If you have questions regarding forms, please email Registrar, Judy Ligon.

Commitment

A condition for participation in athletics is the Athletic Team Pledge.  This agreement will be discussed at the parent meetings in the fall and with students at the first day of practice.  We ask students to acknowledge their understanding of the athletic team pledge and participation in a team sport at Presbyterian School by signing the pledge. We feel that the minimal requirement for athletic participation is to attend practices and games.  While we realize that many variables can occasionally interfere with practice attendance, we need an understanding that clarifies our expectations for commitment to the athletic team. Note to 8th grade parents:  The 8th grade trip to Colorado is August 13-18.  Eighth grade athletes will miss the entire week of August 14-18 as a result of this trip, and will return to practice on August 21. Therefore, it is very important for 8th grade participants to attend the earlier practice sessions.

Athletic Team Moms
Each team needs a team mother to arrange team snacks, etc.  If you are interested in assisting, please contact the coach to volunteer.
